New Health & Wellness Assessment for people with Disabilities or who receive SSI, SSD or TANF:
In an effort to provide dynamic quality affordable Wellness and Integrative Family Life Care® services and support to people with Low/Fixed Income, we are now offering reduced Wellness rates, with certain restrictions and qualifications for Preventative Healthcare.
If you receive Supplemental Security Income, Social Security Disability or TANF and your income does not exceed the Poverty guide lines below:

You may qualify for 45.00 General Wellness Assessment per visit (Normally 120.00, over 60% discount), plus reduced Wellness lab profiles. You will receive the same quality Integrative Family Life care®.
Note: You must show proof of income, SSI, SSD, TANF and any additional info we request and we will verify your eligibility before you visit for an appointment whether your case is active or not, in order to qualify. Failure to provide requested info will disqualify you from receiving the discounted General Wellness Assessment Rate. Falsifying info in order to obtain discount rates may result in civil prosecution to the fullest extent of the law.
